Insurance Information

Currently at this time the following insurances are accepted for Durable Medical Equipment. We are currently working to expand our availability when it comes to insurance. If there are any other questions or concerns involving insurance please reach us via phone or through email.

All medical equipment orders through insurance must be filled using a prescribing medical professional. All pertinent information for any orders involving insurance will be completed by a physician or practitioner through the forms provided. Once this information has been submitted by a qualified prescribing medical professional, the order can be moved forward by our DME Specialists. Please, allow time to gather the information from the prescribing physician/practitioner so that the request process can be completed properly. If there are any questions or concerns about the status of a current request please contact our DME Specialists via phone or through email.

To determine if an insurance policy will cover specific medical equipment:

  • If you are unsure about the insurance coverage of a specific item that you are trying to request, contact your insurance provider to receive accurate information relating to their current coverage.

This is a general list of requirements for order fulfillment:

  • Face to Face documentation - face to face notes will address reasons for necessity while also showing direct conversation with a certifying practitioner, physician or qualified non-physician practitioner (NPP). These are sent from the notes of the prescribing physician or practitioner.
  • An up to date list of recently prescribed durable medical equipment or supplies.
  • A clear scan of front and back of insurance card providing the patient's name, date of birth, and insurance policy number.
  • All forms filled and sent over from the prescribing physician or practitioner.

Q1. What does my insurance cover?

Reach out to your insurance provider to see if the item you are requesting is covered by your insurance.

Q2. What do I have to fill out to start the process of requesting DME or supplies?

Before beginning the process of requesting DME and/or supplies, verify with your insurance provider to determine if the item being requested is covered through your insurance plan. Once insurance coverage is verified, contact a prescribing physician or practitioner to address the need or necessity of DME and/or supplies. The prescribing physician/practitioner will begin the process of collecting the materials necessary to show medical necessity to send to our DME Specialists. We cannot complete any orders without completing everything shown in the drop down titled "What We Need for Orders".

Q3. What insurances do you provide DME and/or supplies for?

  • Aetna
  • Blue Cross Blue Shield (Anthem)
  • Cigna
  • Health First Colorado (Medicaid)
  • Humana
  • Medicare
  • United Healthcare

Q4. What is Medicare's defintion of DME?

  • Durable (can withstand repeated use).
  • Appropriate for use in the "home" ("primarily used at home," but not exclusively).
  • Primarily and customarily needed for a medical purpose (the equipment/supplies would not be useful for someone who doesn't need it).
  • Necessary and reasonable for treatment of a condition or injury.

Q5. Does Golden Gate Medical Supplies do repairs?

We do not specialize in repairs but have been known to do simple repairs to items such as replacing brakes, wheels, accessories, etc. Repairs are to be done at the discretion of the patient through a certified service.

Q6. What are the items that are covered by Medicare?

  • Hospital Beds
  • Manual Wheelchairs
  • Crutches, Canes, Walkers, & Commodes
  • Blood Sugar Monitors and Test Strips

Q7. What criteria is generally needed to qualify for DME Coverage?

  • The beneficiary is enrolled in Medicare Part B
  • The need for the DME items are documented by a practitioner who certifies medical necessity
  • Physician or practitioner completes an order and fills out necessary forms to show medical necessity of DME and/or supplies
  • After a face-to-face meeting with the treating physician or practitioner (certifications via telehealth are permitted but may be subject to certain limitations)

Q8. Is it possible to rent DME through insurance?

Insurance determines if an item is a purchase or a rental. If the item is deemed a rental, the time limit approved for the item is designated through the insurance. To know if the item you are requesting will be a rental or purchased item, contact your insurance provider to discuss DME item coverage. Please Note: We are not currently doing wheelchair rentals through insurance based programs. We will however do out of pocket payments through a rent-to-own payment plan. Q9. If I am receiving consistent supplies/equipment from a certified DME supplier, what life changes do I need to inform the certified DME supplier about?

  • Changing insurance companies
  • Changing doctors
  • In the hospital or will soon be admitted to the hospital
  • In a nursing home or will soon be admitted to a nursing home
  • Beneficiary or secondary contact information has changed

Q10. Am I able to get DME and/or supplies out of pocket?

If you are looking to get medical equipment or supplies, you do not have to go through a prescriber to get the items you need. Before purchasing any items out of pocket, speak to a doctor or physician or practitioner to make sure you are getting the right items for the needs that you have. Ask about our unique rent-to-own programs so that we can help make the process more affordable.

Please note: All relevant information must be included to complete the process of authorization. If any forms are incomplete processing of requests cannot be completed on our end. If the form that is needed is not located below, submit the proper form or contact us so we can find the correct form.
